All new clients must fill out this form before you will be allowed to book your pet’s stay. The information on this form will go into your pet’s file with us so it is important that all information is accurate. All pets must be fully vaccinated within the last 12 months. In an emergency, if the owner is unable to be reached, the owner agrees that veterinary treatment may be sought in emergencies without prior consultation of the owner.
